About the Position
We have a Full-Time opening for a Veterans Affairs Supportive Housing (VASH) Case Manager position in Seattle, Washington. A successful applicant will become a member of a dedicated team whose mission is to work with houseless Veterans and their families to coordinate and monitor successful community living through the HUD-VASH voucher program. Specifically, this position helps connect Veterans with qualified housing and maintain that housing as they stabilize in other areas of their lives. This may include assisting them with obtaining benefits, helping them gain independent living skills, interfacing with landlords, accessing public transit and services, and connecting them to services that help with maintain sobriety and improve their mental health.
The VASH Case Manager position includes work throughout the community and has the ability to complete some work from home.
A company vehicle and supervision by an LICSW for licensure hours is provided.
Work Schedule: Monday through Friday, 8:00am - 5:00pm (Full Time, Day)

About the Program
VASH works directly with Veterans by assisting them in navigating very complex systems (rental applications and requirements, HUD subsidy applications, benefits systems, public transportation, etc.), providing other supportive assistance, helping them maintain residence, and teaching skills to help them become as stably housed and independent as possible in the community.

What You'll Need

Physical Requirements: This position requires the ability to frequently talk, listen, use hands and fingers, reach, stand, walk, sit, bend, stoop, climb stairs, as well as lift, carry, push, and pull up to 40 pounds. It may require the ability to occasionally use ladders, squat/kneel, and perform other physical tasks as applicable. In addition, you must successfully complete crisis de-escalation and CPR trainings, which require performing various physical, hands-on maneuvers and techniques. Duties & Responsibilities
Independently conduct regular sessions through various modalities (virtual sessions, virtual reality, etc.) for the assigned caseload to:
Provide individualized parent education
Supervise the implementation of the program
Monitor the completion of all necessary documentation
Provide in-field training to each direct report
Educate families on the importance of maintaining a consistent schedule with number of hours that are clinically appropriate for the client
Efficiently and effectively communicating CSD's policies to families
Complete necessary documentation including:
Treatment plans
Data sheets
Behavioral support plans
Progress reports and discharge reports
Other site-related documents
Monitor daily direct service appointments for:
Completion; i.e. rendering and accurate notes.
Behavior Specialist cancelations
Family cancellations
Maintain appropriate progress notes for supervision hours and input MLS/HLS supervision hours on CSD's billing program.
Maintain up-to-date records of CSD's templates, forms and other documents
Write, sign and submit progress reports to funding sources by the established deadlines. As a result, is responsible for maintaining a database of all report deadlines for assigned caseload. (Each insurance company will have its own deadlines. Deadlines are available on the policies CSD has created for each insurance company.)
Meet daily and monthly billable hours goal as set by CSD.
Provide clinical support and training to assigned team of Clinical Leaders and Behavior Specialists by:
Holding monthly 1:1 and team meetings and appropriate trainings
Creating individualized clinical trainings
Facilitating and completing trainings as assigned by the Regional Director
Assuring that supervisees' clinical responsibilities are defined and understood as deemed by CSD's Commitment to Quality Standards.
Evaluating clinical performance of supervisees and communicating concerns to the assigned Regional Director
Maintaining and documenting appropriate progress notes for supervision hours
Supervise and guide staff/supervisees with all clinical processes including but not limited to:
Behavioral support plans
Assessments
Submission of monthly session plans
Maintenance of monthly log of lesson plans
Completion of clinical reports and data collection
Maintain regular communication with Human Resources and the Regional Director regarding administrative concerns involving clients, Behavior Specialists, and/or Clinical Directors.
Remains current regarding new research, current trends and developments related to autism, special education, and related fields.
Attend staff meetings, in-services, trainings and other meetings as requested.

JOB SUMMARY:
Provide support services to review and coordinate clinical utilization and external payer expectations for each patient admission where external payment options exist.
Serve as a clinical resource person for the health care team when determining appropriateness of admission and continued stays.
Gather required clinical information and report to insurance company’s patient’s progress and plan to receive authorization for continued stay.
Maximize certified patient days that are clinically appropriate and available as benefits
Assist with insurance pre-certifications for patients who are admitted during regular shift hours and follow up on pre-certifications that are performed or initiated during off-shift hours.
Appeal denials of certified days when it is in the best interests of both the patient and Valley Hope to do so.
Effectively communicate patient status information with the business office, nursing, and counselor.
Regularly review medical necessity guidelines for managed care partners.
Communicate information to appropriate staff members to assist in discharge planning efforts
Provide effective documentation to Valley Hope contract physicians and/or other clinical staff to assist with peer to peer reviews.
Inform UR Manager of any ongoing trends/issues occurring with managed care companies.
Completes any required continuing education credits.
Maintains current state license/certification.
