Provides mental health assessment, diagnosis, treatment and crisis intervention services for adult and/or child members who present themselves from psychiatric evaluation with a broad range of mental health needs. Collaborates with treating physician, psychiatric and allied health professional team to plan and direct each individual member treatment program.
Essential Responsibilities:
Develops, implements, coordinates, and evaluates clinical treatment programs for the diagnosis, treatment, and/or referral of Health Plan members with acute or chronic mental illness. Participates in staff conferences to select, plan, and evaluate treatment programs. Provides outpatient psychotherapy to individuals, couples, families and groups.
Instructs and counsels patients and their families regarding compliance with prescribed therapeutic regimens and adherence to prescribed medication regimens, within the scope of practice. May administer specialized therapeutic procedures, as appropriate. Provides appropriate support to members family. May develop and conducts psychoeducational classes and groups.
Prepares intake summaries, treatment plans, and case summaries and maintains ongoing confidential records. Charts members treatment and progress in accord with state and NCQA regulations and in keeping with accepted community standards. May be required to participate in the department on-call rotation.
Collaborates with physicians in screening and evaluating patients for psychotropic medications, within the scope of practice. Utilizes resources of public and private agencies and community organizations to meet the needs of the members treatment. May develop, implements, and evaluates behavioral medicine and health psychology programs in a variety of settings, including primary care. Provides consultation to other care providers and health educators on matters relating to mental health, health psychology and behavioral medicine.
May supervise Post Masters Fellows, Associate Clinical Social Workers, Associate Marriage Family Therapists or Associate Professional Clinical Counselors as needed if supervision course is completed.
May provide appropriate support to members family, including explanation of treatment, instructions in how to support treatment and interventions to increase acceptance of and adherence to treatment, at members request.
Utilizes resources of public and private agencies and community organizations to meet the needs of the members treatment to include referral of the member and/or members family to external resources, as appropriate. Participates in departmental program development, implementation and evaluation.
Reports safety concerns to mandated reporting agencies.

Position Summary
KIND seeks a Senior Social Services Coordinator to serve alongside the Social Services Management team. Senior Social Services Coordinators provide therapeutic support to high-risk cases, clinical consultations, and contribute to the design and implementation of trauma informed outreach and training within KIND and to external partners. Essential Functions
Supports the development of programmatic framework for Social Services including metrics, logic models, and narratives, for grant applications and reports.

Qualifications and Requirements
Graduate degree in social work or related field or 6 years of work experience, combined with Undergraduate degree in social work or related field, in lieu of advanced degree.
Depending on location - at least 2 years of experience working with Tender Age children, foster care, at risk youth.
Minimum of 3 years of experience in social services or non-profit organizations; community organizing, or grassroots and partnership development experience preferred.

POSITION SUMMARY: The TAY Case Manager provides intensive, weekly case management for transition age youth (TAY) ages 18-24 who experienced homelessness and are referred from San Francisco's Coordinated Entry System to First Place for Youth's TAY Rapid Re-Housing (RRH) Program. Through comprehensive case management, the TAY Case Manager helps youth to identify and attain goals in the areas of self-sufficiency, independent living, health and wellness, relationships, employment and education, and personal advocacy, with a keen focus on housing procurement, retention, and long-term stability. The Case Manager also provides community-based services throughout the community and at the youth's home to teach and model independent living skills development. This position works within a team of Housing Coordinators and a Resource Specialist ensuring housing procurement and connection to community resources. This position is required to work onsite and, in the community, primarily in San Francisco, Alameda and Contra Costa Counties, Monday - Friday.

Responsible for providing case management and skills development to participants

INTRODUCTION
This is an in-person position located at the J-Sei office, 1285 66th Street, Emeryville, CA. This position requires a self-motivated person to work independently under general supervision and within a team to provide case management services to assist older adults and their families.

RESPONSIBILITIES
Perform home visits and psychosocial assessment of the clients, develop, implement, care plan and monitor and follow-up with client in a confidential manner
Uses own car to conduct home visits, attend meetings/appointments and potentially on a rare occasion transport clients to appointments
Provide information, assistance, referrals and follow-ups to older adults and their families regarding senior-related community resources
Maintain client registration and intake records in accordance with agency policy and funding requirements
Write clear, concise progress notes and submit monthly reports in a timely manner
Offer information and assistance over the phone
Provide assistance in agency's community outreach
Participate in staff meetings, and community meetings, as appropriate
Contribute case management related articles to J-Sei newsletter
Perform other related duties as required.
REPORTING RELATIONSHIP
The Director of Services provides overall supervision of the position. Evaluations are conducted by the Director of Services.
QUALIFICATIONS
1. Minimum of two years of experience working in the field of aging or the provision of senior services.
2. Bachelor's degree in Social Work or related discipline desireable. Master's degree in Social Work or Gerontology preferred.
3. Knowledge and experience of working with Word, Excel and Google docs.
4. Collaborative approach to achieving goals.
5. Strong communication, listening and people skills necessary to project and promote the
J-Sei mission to seniors, volunteers and general community.
6. Ability to multi-task and prioritize multiple day-to-day operations in a fast paced environment.
7. Familiarity in working with Japanese seniors preferable
8. Ability to speak Japanese desirable, but not required
9. Ability to occasionally move files and supplies up to 25 pounds
