Under supervision of a Licensed Clinical Social Worker, Licensed Marriage Family Therapist or a Licensed Professional Clinical Counselor, provides mental health assessment and triage, diagnosis, treatment and crisis intervention services for adult and/or child members who present themselves for psychiatric evaluation with a broad range of mental health needs. Under supervision, may collaborate with treating primary care providers and with psychiatric and allied health professional team members to plan and direct each individual patients treatment program.
Essential Responsibilities:
Confers and consults with supervisor regarding members progress and treatment and may consult with psychiatrist as well, if both are involved in members care.
May provide appropriate support to members family, including explanation of treatment, instructions in how to support treatment and interventions to increase acceptance of and adherence to treatment, at members request.
Utilizes resources of public and private agencies and community organizations to meet the needs of the members treatment to include referral of the member and/or members family to external resources, as appropriate. Participates in departmental program development, implementation and evaluation.
Reports safety concerns to mandated reporting agencies.
Develops, implements, coordinates, and evaluates clinical treatment programs for the diagnosis, treatment, and/or referral of Health Plan members with acute or chronic mental illness. Participates in staff conferences to select, plan, and evaluate treatment programs. Provides outpatient psychotherapy to individuals, couples, families and groups. Instructs and counsels patients and their families regarding compliance with prescribed therapeutic regimens and adherence to prescribed medication regimens, within the scope of practice. May administer specialized therapeutic procedures, as appropriate. Provides appropriate support to members family. May develop and conducts psychoeducational classes and groups.
Prepares intake summaries, treatment plans, and case summaries and maintains ongoing confidential records. Charts members treatment and progress in accord with Kaiser Permanente, state and NCQA regulations and in keeping with accepted community standards.
Collaborates with physicians in screening and evaluating patients for psychotropic medications, within the scope of practice. Utilizes resources of public and private agencies and community organizations to meet the needs of the members treatment. May develop, implements, and evaluates behavioral medicine and health psychology programs in a variety of settings, including primary care. Provides consultation to primary care providers and health educators on matters relating to mental health, health psychology and behavioral medicine.

Position Summary
KIND seeks a Senior Social Services Coordinator to serve alongside the Social Services Management team. Essential Functions

Qualifications and Requirements
Graduate degree in social work or related field or 6 years of work experience, combined with Undergraduate degree in social work or related field, in lieu of advanced degree.
Depending on location - at least 2 years of experience working with Tender Age children, foster care, at risk youth.
Minimum of 3 years of experience in social services or non-profit organizations; community organizing, or grassroots and partnership development experience preferred.

POSITION SUMMARY: The TAY Case Manager provides intensive, weekly case management for transition age youth (TAY) ages 18-24 who experienced homelessness and are referred from San Francisco's Coordinated Entry System to First Place for Youth's TAY Rapid Re-Housing (RRH) Program. The Case Manager also provides community-based services throughout the community and at the youth's home to teach and model independent living skills development. This position works within a team of Housing Coordinators and a Resource Specialist ensuring housing procurement and connection to community resources. This position is required to work onsite and, in the community, primarily in San Francisco, Alameda and Contra Costa Counties, Monday - Friday.

Responsible for providing case management and skills development to participants

Minimum Qualifications:

About the Role: This role is a combination of Outbound Product Development and Developer Advocacy. As a Product Advocate for Infrastructure Software for AI, you will be the Directly Responsible Individual (DRI) for outbound product tasks including positioning, competitive analysis and Go-To-Market Strategy based on deep engagement with developers and enterprises. Your work shapes the product launch and strategic differentiation and is directly input to the senior management. Another Road to Safety Program (ARS) is an early intervention and prevention program offered through the Prevention and Intake Division of the Alameda County Department of Children and Family Services (DCFS or CFS). The strength of the ARS Program is our staff. Our Family Support Specialists are clinically inclined and provide some personal experience successfully navigating multi-systems within the Alameda County. Family Support Specialist utilize their clinical expertise to engage families while using a family-centered, early intervention and prevention approach.
Role:
The Family Support Specialist will work closely with families who are at risk of child abuse and neglect in Alameda County. The Family Support Specialist will work closely with families to increase involvement, setting goals, coaching family members through the process, and supporting them in achieving the set goals. The Family Support Specialist also helps to ensure that the integrity of the individual family's culture is fully integrated into every aspect of the care and support offered to the child and family. The Family Support Specialist ensures that culturally sensitive and family friendly engagement practices permeate throughout all of our program offerings. In addition, the Family Support Specialist will participate as a member of an Inter-Disciplinary team; including working closely with mental health professionals while representing the parent’s perspective. As a Family Support Specialist, you will receive support from the Another Road to Safety Program management team in the form of training, professional development, reflective clinical supervision, and administrative support.
Qualifications:
Minimum Qualifications for FSS: A Bachelor’s degree or four years’ experience working in the Human Services or closely related field and previous case management experience.
