Job Duties includes:
Assists patients and families in coping with problems associated with severe and long-term illnesses. Conducts patient and family interviews, prepares psychosocial assessments, develops treatment plans, provides counseling and crisis intervention, and directs patients to designated community agencies and resources. Provides consultation to patient treatment team members and participates in developing new patient care programs throughout Saint Joseph Health System cardiopulmonary and vascular rehab. Provide individual, family, and/or group treatment as part of interdisciplinary treatment plan. Provide quality patient care considering age specific, developmental, cultural, spiritual, diversity, and/or other special needs or circumstances through competent clinical practices. Work amongst a multidisciplinary team specializing in cardiopulmonary and vascular care. Hours variable, Monday - Thursday.
1. Interviews patients and families to obtain psychosocial data.
2. Evaluates and gather data from the patient, family, outpatient supports and other collateral sources (including the primary care provider) regarding plan of treatment and available resources, and develops an appropriate intervention plan.
3. Provides a variety of direct services and clinical interventions in order to provide continuity of care and to help patients and families resolve socio-emotional problems associated with adjustment to illness, resource needs, mental health problems and a variety of life events and transitions.
4. Demonstrates ability to make appropriate and useful changes in the patient's treatment plan when problems persist and recognizes when discharge and/or transfer of care is in the best interest of the patient.
REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE
Masters Degree in Social Work or Psychology from an accredited graduate school.
Current State of Indiana Master's Degree Social Work License or Limited License Social Work degree.
Six to twelve months related experience preferred

Performs Doppler exam and understands blood flow patterns by Doppler spectrum analysis. Performs the Doppler scan by using pulse, continuous, and independent probes, recognizing normal and abnormal flow patterns (carotid imaging, lower extremities).
What you will do:
Assists the physician in obtaining real time images, demonstrating all structures and recording diagnostic finding.
Able to stand for long periods of time (30-60min/exam) while performing exams.
Performs appropriate imaging procedures and obtains optimum diagnostic data for interpretation by the supervising physician.
Maintains appropriate care of all ultrasound equipment.

Requirements:
Education: Requires the knowledge of theories, principles, and concepts normally acquired through the completion of the degree of PharmD, (preferred), or B.S in pharmacy or successful completion of the foreign pharmacy equivalency exam. Residency is preferred not required.

Employment Type:Full time Shift:Description: Physical Therapist (Family Life Center) - to create a distinctive and exceptional therapeutic space for infants and children that will improve their health and quality of life. Improving access and opportunity to children from poor communities with limited resources will be instrumental in improving health equity and decreasing disparities. This uniquely created area will design and implement programs to improve the individual child's areas of need and increase overall function and participation. Individualized assessments, treatment plans, progress assessments and goals will be monitored at each session with family input and in accordance with each child's abilities towards achieving optimal function and movement and prevention of injury.
What you will do:
*Evaluations are conducted with thoroughness and efficiency, including appropriate tests
and measurements.
* Sets functional, realistic, and measurable goals and the patient demonstrates consistent
achievement of those goals.
* Designs a functional treatment plan that is reflective of prescribed therapy and
incorporates applicable reimbursement issues.
* Re-evaluates and discharges patients within appropriate time frames.
Minimum Qualifications:
Education:
Graduate of an accredited school of Physical Therapy is required.
Experience & Skills:
New Grads and Experienced PT's welcome. Prefer Pediatric experience.
Licensure/Certification:
Must be a Licensed Physical Therapist in the State of Florida is required.
