What You'll Do:
Provide on-site behavioral health services in a primary care clinic.

Qualifications
Clinical Psychologist Requirements:
Education: Doctor of Philosophy (Ph.D.)
or Doctor of Psychology (Psy.D.) in Clinical Psychology.

The Behavioral Health Consultant (BHC) provides expert integrated care to pediatric and adult patients as a member of the Primary Care Medical Home or Specialty Care treatment team. They specialize in brief, evidence-based, solution-focused assessments and interventions geared toward functional improvement. BHCs are generalists, providing high volume services from a population-based lens that are accessible, team-based, and a routine part of primary care. This is a behavioral health model that is interconnected with patients medical care and medical care team that requires specialized training and experience with a variety of medical and mental health concerns. In addition, this role performs high-level consultation to physicians and other medical care team members and contributes to a clear treatment plan for patient care. The BHC functions effectively in a fast pace, highly productive setting with the ability to successfully pivot from a patient facing to a physician facing role throughout the day. The BHC thrives on being adaptable, flexible, and able to independently function at a consistently high level. The individual in this position must be willing to engage in brief visits (e.g. 15- 30 mins) at a high volume, and adept in working closely with PCPs and other team members. Other required skills and duties are outlined below.
Essential Responsibilities:
Apply solid working knowledge of biopsychosocial models of human behavior and development for assessment of patient care to include knowledge of the use of adult and pediatric screening instruments, as well as the ability to screen for and identify substance abuse problems.
Apply first line interventions in a time-limited setting for a broad spectrum of physical and mental health problems experienced by patients and families throughout the lifespan as presenting in primary care (pediatrics, Internal Medicine, and Family medicine populations) or Specialty Care (ob/gyn, dermatology, cardiology, etc.) Targets of treatment may include mental health concerns, substance abuse concerns, adverse health behaviors (including their contribution to chronic medical illnesses), stress-related physical symptoms, ineffective patterns of health care utilization, and adjustment to new and chronic medical conditions.
Provide patient and family education effectively and sensitively to encourage compliance with medical and behavioral treatment plans. To include the skill and expertise to discuss common psychotropic medications with a patient, including indications, effects and side effects, and demonstrate the ability to appropriately recommend initiating medication to primary care physician.
Effectively utilize electronic health record and verbal communication to concisely communicate relevant information to medical providers in a timely manner. Ability to concurrently document during patient sessions. To include performance of various clinical, compliance, and administrative duties required to meet various billing and medical documentation requirements.
Effectively assess for and manage suicidal and homicidal risk, as well as concerns for child or elder abuse and neglect, using effective and evidence-based methods (PHQ-9, Columbia Risk Assessment). When applicable, collaborate and coordinate care with the Crisis team, contact police and/or Child Protective Services and communicate effectively with receiving emergency department.
A high level of comfort in providing consultation and training to other members of the primary care team in the areas of mental health, chemical dependency, and behavioral medicine.
Possess skill and expertise to discuss common psychotropic medications with a patient, including indications, effects and side effects, and demonstrate the ability to appropriately recommend initiating medication to primary care physician.

GENERAL STATEMENT OF DUTIES
Consult with other agencies on a wide range of matters related to mental health or alcohol and drug abuse treatment services. The primary responsibility of this class is to provide system coordination and transitions of care in the mental health system with external partners. Coordinate and oversee large community programs, including providing direction to a team of mental health specialists and associates supporting the program. Provide senior-level technical assistance to a Health and Human Services Department program that delivers mental health or alcohol and drug abuse services to the system at-large and senior-level treatment intervention to individuals enrolled in service. Additionally, expectations of positions in this classification are to operate under a high degree of independence and have significant decision-making authority.
SUPERVISION RECEIVED
Work under the general supervision of a manager or supervisor who assigns work, establishes goals, and reviews the results obtained for overall effectiveness through the analysis of reports and conferences.
SUPERVISION EXERCISED
May provide lead direction to teams of other mental health specialists and associates for a specific unit or specific task; may provide supervision to practicum students and volunteers as requested by supervisor.
The Choice Model Exceptional Needs Care Coordinator (ENCC) operates on a systems level across multiple service entities to provide services to individuals with complex needs and diagnoses - including serious and persistent mental illnesses, co-occurring substance use disorders, and a variety of medical needs. The Mental Health Specialist 3 is an outward facing position at the state and local levels and is expected to make sound clinical decisions with complex cases and operate with a high degree of independence.
Hospital Liaison & Care Coordination
Independently act as a liaison between Adult Behavioral Health (ABH) and various stakeholders involved in mental health care, including Oregon State Hospital (OSH), acute care hospitals, coordinated care organizations (CCO), Aging and People with Disabilities (APD), Intellectual and Developmental Disabilities Services (IDD), other programs within Marion County Health & Human Services, etc.
Collaborate with individuals in the mental health system and stakeholders to assess individuals’ needs, determine eligibility for services, appropriateness of services, and authorize enrollment for Choice Model services.
Attend OSH Interdisciplinary Treatment (IDT) meetings for individuals.
Participate in Ready to Transition determination with OSH IDT. Facilitate community transition upon discharge.
Provide case specific consultation to providers and community partners, including clinical recommendations for treatment, services, and risk reduction.
Collaborate with acute care settings and act as CMHP delegate in authorizing requests for Long-Term Psychiatric Care
Track utilization data of those admitted to the Oregon State Hospital and acute care hospitals and provides routine reports to Marion County Management.
Licensed Care/Residential Coordination
Assess individuals’ needs, determine eligibility for services, appropriateness of services, and authorize licensed care/residential placements.
Identify and coordinate placements in appropriate licensed care settings within mental health and other systems (i.e., IDD, APD) for individuals discharging from OSH, acute care hospitals, and/or residing in the community. This includes negotiation with Oregon Health Authority (OHA) to advocate for Rate Reviews for complex cases.
Oversee completion of LSI and LOCUS assessments for licensed providers in order to support rate setting for licensed care.
Organize, facilitate, and/or attend person-centered interdisciplinary team (IDT) treatment planning meetings for individuals enrolled in Choice Model services in collaboration with providers and agencies involved in individuals’ care.
Authorize funding for Choice Model services and licensed care placements with supervisor guidance.
Assess and identify individuals’ readiness to step down to lower levels of care/placement and service needs and gaps.
Monitor residential capacity, track referrals, placements, placement/discharge dates, and utilization data.
Provide clinical consultation in response to unusual incidents, grievances, and complaints within the residential system; this includes bi-annual training to providers on topics related to mental health.

Log, track, and report Trial Visit of State Hospital data to Marion County Pre-Commitment Unit.
Monitor adherence to Trial Visit conditions for OSH discharges within Marion County. Coordinate with the court system to complete Trial Visit revocation for those who violate these conditions.
Coordinate Transfer of Jurisdictions between counties as needed.
Assessment & Treatment Planning
Complete intake and annual comprehensive mental health assessments, develop individualized treatment plans, develop crisis prevention plans, and direct provision of treatment services.
Provide therapeutic intervention and intensive services to individuals, requiring advanced clinical skills.
Documentation
Maintain accurate and timely clinical documentation in accordance with Federal, State, County, and treatment program requirements under the direction of the clinical supervisor.
Meet performance expectations for time and billing standards.
Compile and submit Choice Model reports to the Oregon Health Authority.
Miscellaneous
Participate fully as a member of the Marion County Adult Behavioral Health team as a supportive, reliable, positive, collaborative, and professional team member.

What You'll Do:
Provide on-site behavioral health services in primary care clinic.
Provide consultation with Providers to aid

Qualifications
Clinical Psychologist Requirements:
Education: Doctor of Philosophy (Ph.D.)

The Behavioral Health Consultant (BHC) provides expert integrated care to pediatric and adult patients as a member of the Primary Care Medical Home or Specialty Care treatment team. They specialize in brief, evidence-based, solution-focused assessments and interventions geared toward functional improvement. BHCs are generalists, providing high volume services from a population-based lens that are accessible, team-based, and a routine part of primary care. This is a behavioral health model that is interconnected with patients medical care and medical care team that requires specialized training and experience with a variety of medical and mental health concerns. In addition, this role performs high-level consultation to physicians and other medical care team members and contributes to a clear treatment plan for patient care. The BHC functions effectively in a fast pace, highly productive setting with the ability to successfully pivot from a patient facing to a physician facing role throughout the day. Essential Responsibilities:

Provide patient and family education effectively and sensitively to encourage compliance with medical and behavioral treatment plans. To include the skill and expertise to discuss common psychotropic medications with a patient, including indications, effects and side effects, and demonstrate the ability to appropriately recommend initiating medication to primary care physician.
Effectively utilize electronic health record and verbal communication to concisely communicate relevant information to medical providers in a timely manner. Ability to concurrently document during patient sessions. To include performance of various clinical, compliance, and administrative duties required to meet various billing and medical documentation requirements.
Effectively assess for and manage suicidal and homicidal risk, as well as concerns for child or elder abuse and neglect, using effective and evidence-based methods (PHQ-9, Columbia Risk Assessment). When applicable, collaborate and coordinate care with the Crisis team, contact police and/or Child Protective Services and communicate effectively with receiving emergency department.
A high level of comfort in providing consultation and training to other members of the primary care team in the areas of mental health, chemical dependency, and behavioral medicine.
Possess skill and expertise to discuss common psychotropic medications with a patient, including indications, effects and side effects, and demonstrate the ability to appropriately recommend initiating medication to primary care physician.

What You'll Do:
Provide on-site behavioral health services in a primary care clinic.
Provide consultation with Providers to aid
or assist in the primary care of patients.
Serve as primary mental health Provider
or ancillary health Provider as needed.
Assist care team with ICD-11 diagnoses, adding mental health differential diagnoses and contextual and relational perspectives.
Work across disciplines to provide equitable biopsychosocial-spiritual care, intervening in support of team and patient goals.
Use culturally informed and evidence-based literature for shared decision-making around health behaviors and assist with navigating the healthcare system.
Work in a closely organized, interdisciplinary team on treatment plans, patient advocacy, and clinic processes.
Conduct individual and group training for staff in areas of expertise and to community organizations as requested by supervisor.
Develop research-related funding proposals.
Partner with other Providers to triage referrals.
Provide coverage and backup for other Providers.
May assign patients to team clinicians.
Perform other duties as assigned.
Perform clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), American Psychological Association (APA) and YVFWC requirements.
Represent the program at meetings as requested by Behavioral Health
or clinic leadership.
Participate in the development of new programming and projects related to Behavioral Health
Qualifications
Clinical Psychologist Requirements:
Education: Doctor of Philosophy (Ph.D.)
or Doctor of Psychology (Psy.D.) in Clinical Psychology.
Experience Preferred: One year's clinical experience working within a multidisciplinary team in primary care
or other medical settings, providing diagnostic and psychological assessments for primary care-based positions. One year's clinical experience working with children, adolescents, and families in specialty behavioral health settings.
Licenses/Registration: Licensed Psychologist within the state of practice. Basic Life Support (BLS) certification within 45 days of hire.
Master's Level Therapist Requirements
Education: Master's Degree in Social Work, Psychology, Counseling,
or related field.
Experience Required: Two years of postgraduate/master's experience in the direct treatment of persons with mental illness
or emotional disturbance; such experience must have been gained under the supervision of a mental health professional. Washington State designation as a Mental Health Professional (MHP) may be substituted for this experience requirement.

Addiction Medicine (AM) Counselors provide engagement, counseling, and case management services to members that are struggling with substance use disorders. AM Counselors are generalists who are flexible in their job duties serving a population that has a variety of needs including treatment for substance use disorders, co-occurring mental health disorders, and support and education for their family members. AM Counselors function within a multidisciplinary treatment team to support members that present in all phases of readiness for change.
Essential Responsibilities:
Provide high quality clinical services (individual, group, and family) to our adult and adolescent members through screening, assessment, diagnosis, and formulation/implementation of service plan in outpatient or residential settings. Provide all care in accordance with accepted best practice standards and department protocols (including supervision). Provide case management through appropriate triaging and referrals, consultation with other providers, contact with members and their families, and coordinating with outside agencies. Coordinate care for patients receiving medications from the Addiction Medicine Medical Team.
Document all member care in the electronic medical record in accordance with accepted medical/legal standards.
Provide other member or department services, such as program development, in-service training, and participation in process improvement activities.

Job Duties:
The School Counselor supports students and their families to ensure positive social interactions and academic success. The School Counselor is expected to collaborate with teachers, parents, administrators and other school staff to create a safe, healthy, and supportive learning environment for all students. Consultation:

Report to crisis situations in tandem with colleagues and community personnel.

Develop accurate data tracking systems for accountability and timely interventions cycle as part of the SSP process.
Prevention:
The City View Counselor is responsible for delivering a comprehensive guidance program for all students in grades 6-8.
In addition, the school counselor will work with individual students who are experiencing specific challenges ranging from social skills, decision making, goal setting, peer relations, and self-regulation.
Work with the whole classroom and small groups of students on general guidance topics of concern to the school community, such as bullying, conflict resolution, self-regulation skills, and diversity awareness.
Identify and/or develop programs for children at risk of failing at school, based on needs assessments.
Implement and deliver programs and strategies to improve school climate, based on consultation, collaboration and observation at all grade levels.
Maintain regular communication, education, and outreach to strengthen the school to home connection.
Support and understand the EL Education model that our school implements.
Provides in-service training to faculty and parents as needed.

Participating in a summer staff development institute or retreat in August.

Open to restricted counseling license if eligible.
