Essential Responsibilities:
Plans, develops, implements and maintains social services policies and procedures, delivery of social work programs, and discharge planning services.
Oversees social services case management activities.
Manages staff activities, ensures their competencies, and develops/presents inservices.
Recommends, develops, and implements new programs enhancing services to specialized patient populations and reducing hospital and office visit utilization.
Develops and manages departmental budget.

Identifies community resources for our members.
Collaborates with community organizations in developing new programs or enhancing current community services which are available to Kaiser members.
Develops, implements, and monitors the quality assurance of the social services programs and staff.
Identifies issues which interfere with the delivery of services and develops techniques to resolve them.
Collaborates with medical center departments, community agencies and health plan members on matters related to discharge planning and ensures that services are provided in an integrated fashion.
Ensures compliance with federal, state, and local requirements.

Basic Qualifications: Experience

About the Opportunity
As a Behavior Specialist at CSD, you'll play a key role in supporting children with autism. Using strategies backed by science (called Applied Behavior Analysis, or ABA), you'll help kids build important life skills-like communication, social interaction, and daily routines-in their homes, at our centers, or in community settings.
You'll follow a personalized plan created by our clinical team and bring energy, compassion, and structure to each session. Whether you're playing games, guiding through tasks, or celebrating a small win, your support helps children grow in big ways.

Duties & Responsibilities
Conducts 100% of scheduled sessions, unless time-off has been granted or the parents of the client have communicated to the clinical team that they would not like for the hours to be provided.
Primarily responsible for conducting subbing sessions for other clients to sustain the productivity margin if a session is cancelled by the family. Subbing sessions will fall on Saturdays and within the employee's availability.
Additional hours or sessions may be assigned by the Regional leadership within employee's availability.
Implements individualized treatment goals and Behavioral Intervention Plans in response to the direction provided by the Clinical Director and Senior Clinical Leader/Clinical Leader and/or other clinical staff in order to improve the behavior and skill acquisition of clients diagnosed with Autism Spectrum Disorder.
Travels to Center, Home, and other community settings to deliver services.
Subbing sessions will primarily take place in the regional centers
Renders completed sessions daily and writes thorough notes
Communicates schedule changes and cancellations to clinical team immediately.
Participates in regular meetings and promptly communicates with Clinical Leaders/Clinical Directors via email and phone, regarding Treatment Plans and Behavioral Intervention Plans, family concerns and questions.
Fulfills the timely completion of necessary clinical documentation and communication of:
Daily Treatment Session Notes
Daily Treatment Target Data
Daily (If applicable) Behavior Data.
Keeps updated with other site-related documents and current clinical reports for each child served.
Remains current regarding new research, current trends and developments in special education and related fields.
Attends staff meetings, in-services, trainings, and other meetings as requested.

Under the direct supervision of the Residential Services Supervisor (RSS), the Residential Counselor II is responsible for the care and supervision of the severely emotionally disturbed children (residents) assigned to the facility in accordance with the mandates of public law, state regulations and Agency operating plans. The Residential Counselor II acts as the facility manager and assists the RSS/STRTP Administrator in the management of the residential facility.
Essential Functions:
Ensures the safety of youth through direct supervision.
Oversees the implementation of trauma informed interventions with all youth and can effectively manage crisis.
Responsible for responding to crisis and ensuring all required reporting is completed.
Oversees daily activities of youth and ensures implementation of the Short-Term Residential Therapeutic Program's (STRTP) operating plan.
Oversees the passing, documenting, and monitoring of the use of medications by youth.
Oversees facility documentation such as Special Incident Reports (SIR), Shift Summaries, Assures adequate staff coverage for the houses and arranges for emergency coverage.
Participates as an active member of the treatment team and attends assigned events and meetings.
Minimum Required Education and Experience:
Must have a Bachelor's degree from an accredited four-year college or university -or- two years of full-time experience working with the population served -or- two years of experience working in a group home or STRTP.

Job Description
Under general supervision, Protective Service Workers perform professional investigation, case management, and casework services at the Family & Children's Services program. This includes activities related to: engagement of children, youth and families; assessment of safety and risk; case planning and service delivery; and transitioning children and families to appropriate permanent and stable living situations.
ESSENTIAL TASK AND DUTIES
According to Civil Service Commission Rule 109, the duties specified below are representative of the range of duties assigned to this job class and are not intended to be an inclusive list.
1. Partnering with families, communities, and service providers to build a supportive team that addresses the safety, health, and well-being needs of the child, youth, and family.
2. Exploring safety, well-being, family relationships and natural supports through interviewing, field visits, and gathering collateral information in a timely manner.
3. Conducting comprehensive assessments of safety, risk and protective capacity utilizing designated assessment tools. Taking appropriate action to ensure child safety, including promoting family strengths, mitigating safety threats and/or removal of children from the home.
4. Maintaining case information on applicable computerized case management system(s). Collaborating with families and stakeholders to develop behavioral case plans based on the assessment that addresses the family's needs and complies with relevant laws, rules and regulations.
5. Helping connect families to a broad array of services and supports and empowering them to utilize strengths in order to implement the provisions of a case plan.
6. Counseling and educating youth, families, care providers and community partners on the Child Welfare process, legal requirements and departmental policies and procedures by communicating verbally and in writing in order to provide necessary and appropriate information to families.
7. Building community partnerships through relationship building and educating community organizations about Family and Children's Services.
8. Staying abreast of latest policies, initiatives, and best practices by attending trainings, reading literature reviews, accessing various online child welfare resources and participating in work groups and site teams to contribute to the development of policies.
9. Ensuring all assigned cases meet compliance standards and scheduling work accordingly.
10. Testifying in Court and adhering to Court protocol in compliance with legal requirements.
11. Ensuring the well-being of children placed in homes by identifying homes to meet children's needs and closely monitoring their placement and providing assistance to out-of-home care providers so that children go to mandatory parental visits, medical and dental appointments in order to comply with mandatory county and state procedures, rules and regulations.
12. Preparing written correspondence, electronic records and court reports by maintaining well documented records in a clear and timely manner according to agency procedures.
Qualifications
1.
Education:
a.
Possession of a Master's Degree in Social Work from a school accredited by the Council on Social Work education (CSWE);
OR
b.
Possession of a Master's Degree from a two-year (2) counseling program from an accredited college or university. (Qualifying Master's Degrees in counseling include Marriage and Family Therapy, Clinical Counseling, Mental Health Counseling, Addiction Counseling and Counseling Psychology that included a supervised field placement as part of the course curriculum).
AND
2.
Experience:
a.
Satisfactory completion of two semesters of graduate field placement, which involved the provision of direct services to families or children in one of the following or closely related areas: child welfare, family counseling, juvenile probation, pediatric social work, adolescent alcohol and drug counseling or school social work;
OR
b.
Six (6) months post-baccalaureate professional work experience which involved the provision of direct services to families or children in one of the areas previously described.
AND
3.
License:
Possession of a valid California class C driver's license and clear Motor Vehicle Record (MVR).

Street Crisis Response Specialist

The San Francisco Homeless Outreach Team (SFHOT) works collaboratively in small teams to engage and stabilize individuals experiencing homelessness, to help establish treatment, and to find permanent housing.
This Street Crisis Response Specialist role will be assigned to the Street Crisis Reponse Team and is comparable to an Outreach Specialist Level 3 role. Working with community paramedics and EMTs from the San Francisco Fire Department (SFFD) and responding to 911 and 311 service calls, Street Crisis Specialists will be dispatched to focus on a range of calls from overdose medical emergencies and acute behavioral health crisis to less immediate calls and well-being checks that require immediate attention.
The team will perform medical, behavioral, and social needs assessments, render immediate aid if needed, and will be able to offer meaningful connections to services and housing. The team will be deployed in an SFFD vehicle and have the ability to provide transportation services to individuals who might need that as part of the engagement
The work schedule is a 10-hour shifts, 4 days a week. The operations for this project are 24-hour days, 7 days a week. Exact shift hours and days assigned are TBD.
ESSENTIAL FUNCTIONS
Respond to 911 and 311 calls from the community, reducing SFPD response with a focus on well-being checks and situations that require immediate attention, such as an acute behavioral health crisis.
Perform wellness checks, assess risk, provide support, and referrals for needed services. This may include crisis intervention, and engaging emergency medical or mental health systems.
Engage individuals and assess needs (eg, medical, mental health, substance use, shelter, food access, benefits, and other issues). Follow up with clients to support linkage, as needed.
Advocate and collaborate with service providers to facilitate access to care with the goal of stabilization.
Respond to requests from San Francisco Fire Department (SFFD) to help individuals found at-risk in places not meant for human habitation.
Work collaboratively as part of a dynamic and diverse team, which includes clear communication, flexibility, and humility.
Work in close collaboration with the DPH Medical Team (eg, Street Medicine, Shelter Health, Behavioral Health) to link people experiencing homelessness to transitional primary care.
Maintain professional and positive relationships with other service providers, community groups, and the neighborhood.
Must demonstrate ability to work anywhere in San Francisco and work with clients of various backgrounds.
Must attend scheduled clinical and administrative supervision, to discuss client care, service delivery, and clinical and professional development.

MINIMUM QUALIFICATIONS
